Enrollment fee $100 for individuals, $150 for couples, and waived for children with adult membership.
Subscription fee is waived for each child after the 5th membership, immediate family only, residing in the same residence.
*Child – Dependent, living at home or with a valid student I.D.. College student discount available up to age 26 on a case-by-case basis.
Subscription may be terminated with 24 hours notice by the patient.
Charges paid by automatic credit card charge or bank draft.
Billing processed monthly, quarterly, or annually according to patient preference One month subscription discount for annual payment.
